HOW IT WORKS
1. Schedule
Book your appointment online. A non-refundable deposit of $250 is due when your appointment
is accepted to secure your session.
2. Intake
Complete your intake paperwork, sign liability waivers, and upload your police report and any
relevant legal documents.
3. Session
Attend your 60–90 minute Zoom appointment. You'll need access to Zoom on a phone, tablet, or
computer.

4. Report
Receive your completed evaluation within one week of your session, sent directly to you. You
decide who sees it

Who needs this evaluation?
Anyone charged with OUI/DUI in Massachusetts who has received a 24D disposition and been
referred by the RMV as part of their license reinstatement process.


How long does the whole process take?
From your first appointment to your completed report, most people are done within 2–3 weeks.


What do I need to prepare for intake?
You'll complete intake paperwork, sign liability waivers, and upload your police report and any
relevant legal documents before your appointment. Instructions are sent when you book.


Is this evaluation confidential?
Yes. Your report is HIPAA-protected and sent directly to you. You control who sees it — nothing
is shared without your explicit consent.

What does the report include?
A clinical summary and Risk of Recidivism Assessment completed to RMV specifications. It
contains what is required — nothing more, nothing less.


How does payment work?
A non-refundable deposit of $250 is due at the time of booking. The remaining $350 is due at
the time of your session. This is a cash-pay service — insurance is not accepted.
