Substance use evaluations are requested for a variety of reasons beyond OUI/DUI — including by employers, Employee Assistance Programs (EAPs), the Department of Children and Families (DCF), and Probate Court. Each situation is different, and each evaluation is approached accordingly.

WHAT TO EXPECT
Each evaluation is conducted via Zoom and includes a clinical interview and review of any
relevant documentation provided. Timelines, scope, and deliverables are determined based on
the nature of the request. All evaluations are HIPAA-compliant and conducted with discretion.

Fees are determined on a case-by-case basis depending on the scope and requirements of the
evaluation. Payment details will be discussed prior to scheduling. This is a cash-pay service — insurance is not accepted.
